Conditions of Enrollment

The above applicant is registering for a group dog training class to be held by DogPACT.
 

The undersigned acknowledges and understands that dog training involves potential risk of personal injury and injury to personal property including dogs, which may result from the undersigned's participation in this group training class. The undersigned further acknowledges that dog-training activities are potentially dangerous and the undersigned freely assumes all risk of injury and/or damage that may result from any reason whatsoever thereby.

It is understood that enrollment is not effective unless a) payment for the total amount of the class fee is received by DogPACT; and e-mail confirmation is sent to you by DogPACT. Due to the small class size, this is a non-refundable agreement unless we receive notification of intent to drop the class 10 calendar days before the start date of the class, and a $25.00 administrative fee will be deducted from the class fee and the balance refunded.

To the best of my knowledge, my dog has never [aggressively] threatened another dog or a person]. This includes any of the following behaviors: a) growling; b) snapping; c) nipping; d) biting; e) lunging in a threatening way; f) barking intensely and cannot seem to calm down. I agree to retain all financial liability for any injury my dog inflicts on a dog or person in class. Should my dog act aggressively in class toward a person or a dog, I understand and agree that I may be asked to leave the class. In consideration of being allowed to enroll in this class, I hereby release DogPACT, Terry Long, Maryanne Dell, Kelli Knowles, and VCA Rossmoor-El Dorado Animal Hospital and their officers, directors, agents, employees, and interns from all liability for loss, damage or injury to me or my dog, and agree to hold the aforesaid parties harmless from all liability, damage, cost or expense (including attorney fees) arising out of any claim, demand or action based upon any occurrence concerning myself or my dog. The entire agreement between the parties is set out in this page and there have been no oral representations that do not appear herein and no warranties, either expressed or implied, other than the above contained herein.
